Recruitment · a practical working guide

Membership application forms that make joining clear.

Build a membership application, registration or sign-up form around the decisions you need to make. Download an editable template and field-purpose worksheet.

Explain eligibility and fees before the form, ask relevant questions and tell every applicant what happens next.

Membership application form

01/ Choose the joining route first.

Choose the joining route first.

A membership application form gathers the information needed to decide or establish membership. Registration forms and sign-up forms may do the same job, but the process differs when payment, eligibility checks or approval come before confirmation.

Choose the route before choosing the fields. An open community registration, an assessed professional grade and a company-wide membership need different evidence and decisions. State what counts as joining under your own rules.

The Scottish Government’s form-design guidance recommends explaining the process before someone starts, considering eligibility early and providing clear next steps. These are government design principles we adapt here to membership, not measured membership conversion findings.

RouteExplain before application
Open registrationWho can join and when access starts
Paid membershipTotal price, billing period and when payment happens
Assessed applicationCriteria, evidence, decision-maker and expected timing
Organisational membershipThe business joining, authorised contact and included users

02/ Ask what you need to decide.

Ask what you need to decide.

Write a purpose beside every proposed field. If the answer does not affect admission, payment, account access or a defined operational requirement, consider collecting it later or removing it. “Useful for marketing someday” is not a clear reason for blocking an application.

GOV.UK Forms’ published guidance advises avoiding repeated questions, using understandable language and supporting autofill. For membership, that means carrying known information through the journey and distinguishing required fields from optional profile questions.

Do not make a phone number or postal address compulsory just because an old paper form did. Equally, do not remove evidence that your admissions process actually needs. The right form has justified questions, rather than a promised ideal field count.

FieldWhy you might need itWhen to ask

Eligibility evidenceMake the admission decisionOnly for relevant categories
Employer or businessEstablish an organisational entitlementWhere the offer requires it
Billing informationTake payment or issue an invoiceAt the relevant payment step

03/ Adapt the application template.

Adapt the application template.

Download the editable membership application form template. Paste the plain-text file into your document or use it as a specification for an online form. It does not collect or submit applicants’ details.

The template contains prompts for eligibility, fees, contact details, relevant evidence, review and confirmation. Replace the prompts with your actual policies. Use your approved privacy and communications wording; this worksheet does not supply a legal policy.

For a printable membership form, provide enough space to complete answers and a clear return route. For an online form, preserve the same decisions while using appropriate input controls. Keep sensitive evidence out of ordinary email attachments where a secure route is available.

Template structure

  1. 01Before you start: criteria, fee and documents
  2. 02Applicant: name, contact and chosen category
  3. 03Relevant evidence: only what the category requires
  4. 04Payment: collection or invoice route
  5. 05Review: corrections and applicable rules
  6. 06Confirmation: status, timing and contact

04/ Make questions easy to answer.

Make questions easy to answer.

W3C’s design-system guidance recommends labelled inputs, necessary questions, suitable autocomplete and clear action buttons. This is the standards organisation’s design guidance, not a declaration that a form automatically meets WCAG.

Use persistent labels rather than instructions that disappear inside a field. Explain acceptable file types and relevant date or number formats before an error occurs. When someone makes a mistake, identify the affected field and tell them how to fix it without losing their answers.

Try the whole journey on a phone and with the keyboard. Check category changes, long names, a declined payment, missing evidence and a return from the previous step. Ask people unfamiliar with the form to complete it before launch.

Practical form review · 0/6 in place

Resolve the remaining decisions.

05/ Say what happens after submission.

Say what happens after submission.

Give applicants a chance to check and correct their details before the final action. Make the final button describe what it does: applying for review is different from paying and joining immediately.

After submission, show whether the application was received, is awaiting approval or has become a confirmed membership. Give a reference where relevant, a realistic response expectation and a contact for questions. An automated receipt should not imply acceptance.

The membership management guide covers record handling, and the membership committee guide covers delegated admissions responsibilities. Once confirmed, pass the member to onboarding with the reason they joined.

06/ Find where applicants get stuck.

Find where applicants get stuck.

Agree the events before reporting a form completion rate. Form starts, submissions, paid joins and approved memberships are different stages. Compare the same route and cohort, and avoid collecting the applicant’s answers in analytics events.

Form completion rate is completed submissions divided by form starts for the defined cohort, multiplied by 100. An assessment backlog can reduce confirmed joins without changing form completion, so inspect both operational and form-stage reasons.

Review support questions and recurring errors alongside the counts. If people abandon an evidence upload, test the instructions and supported files before removing the evidence requirement. Use membership website services for implementation, and membership software selection for platform decisions.

07/ questions

Your questions answered.

What should a membership application form include?

The relevant category, eligibility and fee information, applicant contact details, necessary evidence, review and declaration, and a clear explanation of what happens next. Justify each field against your actual process.

Is there a free membership application form template?

Yes. This page links to an editable plain-text template you can paste into your own document or use to specify an online form. It includes a field-purpose worksheet and does not submit personal data.

What is the difference between registration and application?

Registration may establish membership immediately when the conditions are met. An application may need assessment, payment or approval before membership is confirmed. Explain your actual process rather than relying on the form’s name.

Can we use this as a club membership form?

Yes. Adapt the categories, activity requirements, fee, relevant eligibility and approval steps. Remove fields that do not serve your club’s process and provide the correct rules and contact details.

How many fields should a membership sign-up form have?

There is no universal correct count. Ask what is needed at this stage, explain why and collect optional profile information later where appropriate. Test completion with real users.

Should submitting the form start membership immediately?

Only if that matches your rules and the required conditions are met. If assessment or approval remains outstanding, confirm receipt and explain the decision process instead of saying the person has joined.
