What it is

The survey that turns lost members into a to-do list.

A member exit survey is a short questionnaire for members who have chosen to leave, captured while the reason is still fresh. It focuses on one thing a satisfaction survey can’t: why they actually left — and what would have kept them.

It matters because retention is where membership economics are won: keeping a member costs a fraction of winning a new one, and a lapsed member already valued you once. The exit survey tells you, specifically, what to fix for the next cohort.

Use the question bank below to build your own, copy the ready template, then act on the results.

Why it earns its place
Fresh reasons
Asked at the moment of leaving, the reason is specific — not the vague memory you get months later.
Cheaper than acquisition
Retaining a member costs a fraction of acquiring one, so every recovered leaver is high-value.
A win-back list
The “would you rejoin?” score hands you a ready, segmented win-back audience.
Fixes the next cohort
Patterns across leavers show the one structural fix that protects future renewals.

Question bank

Member exit survey questions, by theme.

Pick a theme to see the questions that work. Choose a handful across themes — mostly closed, one open — and keep the whole survey under two minutes.

Get the real reason, not the polite one.
01What is the main reason you decided not to renew your membership?
02Was there a single event or moment that made you decide to leave?
03How long had you been considering leaving before you decided?
04Is there anything we could have done to keep you as a member?
Tip: one “main reason” question plus a rejoin score does most of the work — the rest adds texture.
Copy-and-go template

A ready member exit survey template.

Ten questions, mostly closed, two minutes to complete. Copy it into your survey tool and trigger it the moment a member lapses.

Turn answers into renewals

A survey is only worth the action it drives.

Four steps to turn exit-survey answers into a higher renewal rate — the reason we run them in the first place.

01
Ask at the right moment

Trigger the exit survey the moment a member lapses or declines to renew — while the reason is fresh. Keep it short: five to ten questions, mostly closed, one open box.

02
Code the reasons

Group answers into a handful of reasons — price, value, use, experience, circumstance. Patterns across leavers matter far more than any single response.

03
Fix the top driver first

One or two reasons usually explain most churn. Fix the biggest structural driver before touching the renewal email — that is where the retention return sits.

04
Run a real win-back

A lapsed member already valued you once. Use the “would you rejoin?” score to segment a genuine win-back — it recovers a meaningful share of recent leavers each year.

Where do your leavers sit against the benchmark? See membership retention rate benchmarks.
